Body repatriation required document
The documents required for the consular section to issue a death certificate are as follows:
- The deceased’s passport
- Burial permit
- Certified copy of death certificate
- Photocopy of the funeral home’s registration with the Health Department
- Photocopy of funeral invoice
- A notarized letter certifying that the corpse is not contagious
- A notarized letter on the funeral home’s letterhead certifying that the corpse has been duly embalmed and will be transported in a properly sealed coffin.
- The itinerary of the corpse and the name and address of the funeral home that will handle the funeral in Haiti.

Les documents requis pour l’établissement d’un certificat de décès par la section consulaire sont les suivants :
- Le passeport du défunt
- Le permis d’inhumer
- Une copie certifiée conforme de l’acte de décès
- Une photocopie de l’enregistrement de la maison funéraire auprès du Ministère de la Santé (Health Department)
- Une photocopie de la facture des obsèques
- Une lettre notariée attestant que le cadavre n’est pas contagieux
- Une lettre notariée sur papier à en-tête de la maison funéraire attestant que le cadavre est dûment embaumé et qu’il sera transporté dans un cercueil bien scellé.
- L’itinéraire du cadavre et les coordonnées de la Maison funéraire qui se chargera des funérailles en Haïti.

- Itinerè yo pral fè ak kadav la, ak enfòmasyon pou kontakte mezon finrè a ki pral fe antèman an ann Ayiti.
